Returning Patient

If you have previously consulted any of the specialists at SGRH or have been admitted here, you can book an Online or In-Person appointment with the existing SGRH Registration No. with you

If you are visiting to consult for an ailment, disease or Condition for which you have previously consulted, the specialist might want to review your medical history. If you are a Returning or Referred patient you are requested to carry the following ( not limited to) with you when visiting for your consultation

• A latest prescription/ discharge summary or a summary describing your condition.
• Latest Laboratory/ X-Rays, Radiology reports.
• Copies of any Medical records other than SGRH.
